Hello! I am Jacqueline Johnson— Licensed Esthetician, Advanced Lash Artist and Licensed Permanent Cosmetics Artist. I have been in the beauty industry for five years and also have experience in the hospitality industry. By providing your contact information and booking an appointment with Slashed Beauty Parlor you agree to all listed terms and policies. • Deposits are nonrefundable and nontransferable. • I only accept cash or debit/credit cards as payment. A card reader is provided for an easy checkout experience (3.5% transaction processing fee applies to card payments). Pay with cash and save! • No refunds will be issued for any reason. If you are dissatisfied with any service or product, please contact me directly so I can resolve it. • Arriving with a cleansed, makeup-free face is best for application. Please also avoid excessive moisturizers and oils on the day of your appointment. • For lash and eyeliner services, contact lenses must be removed to prevent irritation. • Refill appointments are maintenance on an existing Full Set, not a total reconstruction. At least 40%-50% of extensions must be on both eyes to be considered a Refill. Anything less will be charged as a Full Set. • I do accept clients who have lashes on from another lash artist (select Foreign Refill option). Foreign Refill lashes must be clean, properly isolated, and meet the 40%-50% coverage requirement for Refills. Otherwise, a new Full Set may be needed. • Rest is best during your appointment. Complimentary phone chargers, bottled water, snacks, pillows and blankets are available for the most comfortable client experience! Thank you for your continued support. Cancellation Policy
I operate each day on a tight schedule with back-to-back clients. Please arrive on time to your appointment. I do not charge late fees, but after 15 minutes the appointment is canceled and treated as a same-day cancellation. No shows or cancellations within 24 hours of the scheduled appointment time forfeit their deposit and incur a 25% fee of the service price.
